Ultimately, we will need your address to mail you the permits for your concert. This information is required.

Choose whether you want your phone number, your email address, or both to be visible to other Artists/Locations who wish to contact you about a performance.

Your contact information will only be visible when you indicate, on a following page, that you have available performance times that are not yet booked.
